By requesting direct admission to a Utah college or university, I acknowledge that the Utah Office of the Commissioner of Higher Education will share with the Utah public colleges and universities I select the following personal data:

• Name
• State Student Identification number
• Birthdate
• Address
• Parent and student email addresses
• Parent and student phone number
• High school
• Utah residency
• Cumulative high school GPA
• ACT or SAT scores
• Date and time of application
• IP address

Sharing this data is necessary for the Utah public colleges or universities to complete the admissions process, waive application fees, provide scholarship information, and communicate with me in support of my admission and/or scholarship availability. Data sharing will only be used for those purposes. All data sharing and security procedures for Admit Utah meet the standards required by law as well as current data privacy/security practices for Utah public school districts, colleges, and universities. I further acknowledge that I am not required to allow Admit Utah to use and share this information. If I do not allow Admit Utah to use and share this information, Admit Utah will not be able to process my request for direct admissions to a Utah public college or university. I will not be denied admission and may still apply for admission through each Utah public college’s or university’s campus-based admission and scholarship process.
